The Impact of Colonization by Multi Drug Resistant Bacteria on Graft Survival, Risk of Infection, and Mortality in Recipients of Solid Organ Transplant: Systematic Review and Meta-analysis

The study aims to assess the impact of multidrug-resistant (MDR) bacteria on solid organ transplant recipients (SOTR)’s mortality, infection, or graft-loss. Data from 15,202 SOTR were analyzed, with liver transplant and VRE colonization being predominant. MDR colonization significantly increased post-transplant one-year mortality and mixed-infections across transplant types, but no impact on graft-loss. A higher association was found between carbapenem-resistant Enterobacteriaceae (CRE) or extended-spectrum beta-lactamase (ESBL) colonization with outcomes compared to MRSA or VRE colonization. The study concludes that MDR colonization in SOTR, particularly CRE, is associated with increased mortality.
